Transporter

Drives transportation van to transport passengers over specified routes to local or distant points according to time schedule.

Transportation Aide

Provide escort services and transportation to residents for medical appointments utilizing the ATA bus, van or personal vehicle. Complete trip logs for accurate documentation of services rendered and for billing purposes. Must submit logs daily.

Nurse Aide

Assist nursing personnel in providing non-professional nursing care to residents under the direction and supervision of a professional nurse. Responsible for the care of assigned residents, tidiness of resident room, personal effects and linens. Answer call lights for all residents regardless of assignment, assist co-workers as needed to foster teamwork environment, and assist in the admission and discharge of residents. Complete height, weight and personal possession sheets, including dentures, glasses, and hearing aides.

Restorative Nurse Aide

Assist Therapists and nursing personnel in evaluating and identifying restorative nursing needs. Evaluate and implement restorative programs under the direction and supervision of professional therapists and Restorative Nurse. Determine short term, measurable, realistic goals reflective of resident's condition and prognosis, and change as needed and reassess at least monthly.

Nursing Administrative Assistant

Assist the Director of Nursing in determining the staffing needs of the nursing department. Recommend the number and level of nursing personnel to be employed. Ensure that sufficient number of RNs, LPNs and Nurse Aides are scheduled for each shift to ensure that quality care is maintained and assist with recruitment of nursing personnel.

Licensed Practical Nurse

Provide nursing care to residents under the direction and supervision of a professional registered nurse. Must be immediately accessible to all residents and shall be qualified by training and experience to assure prompt, appropriate action in the event of an injury, illness, fire or other emergency. Provide basic life support, including initiation of CPR to a resident who experiences cardiac arrest prior to the arrival of Emergency Medical Services based upon the resident’s advance directives or in the absence of advance directives or a DNR order and follow proper reporting procedures of suspected abuse.

Restorative Services Nurse

Assist therapists in evaluating and identifying restorative nursing needs. Evaluate and implement restorative programs under the direction and supervision of professional therapists. Update care plans as needed and communicate changes with Care Plan Coordinator. Participate in resident care plans as appropriate.Determine short term, measurable, realistic goals reflective of resident's condition and prognosis, and change as needed and reassess at least monthly.

Registered Nurse

Provide nursing care to residents and direct and supervise Licensed Practical Nurses and Nurse Aides. Shall be immediately accessible to all residents and shall be qualified by training and experience to assure prompt, appropriate action in the event of an injury, illness, fire or other emergency. Shall provide basic life support, including initiation of CPR to a resident who experiences cardiac arrest prior to the arrival of Emergency Medical Services based upon the resident’s advance directives or in the absence of advance directives or a DNR order.

Infection Preventionist

Will develop and oversee the Infection Prevention and Control program including establishing goals and priorities for the program, planning, and implementing strategies to achieve these goals. Will monitor the implementation of the program (including the IDT infection control practices) and will respond to any identified issues or concerns.

Assistant Director of Nursing

To assist the Director of Nursing in planning, organizing, developing, and directing the day-to-day functions of the nursing department in accordance with current federal and state standards, guidelines and regulations as directed by the Administrator, Medical Director, and/or the Director of Nursing to ensure that the highest level of quality care is maintained. The individual is delegated the administrative authority, and accountability necessary for carrying out the assigned duties. In the absence of the Director of Nursing the individual is charged with carrying out the resident care policies established by the facility.

Director of Nursing

To plan, organize, develop, and direct the day-to-day functions of the nursing department in accordance with current federal and state standards, guidelines and regulations as directed by the Administrator and Medical Director to ensure that the highest level of quality care is maintained. The individual is delegated the administrative authority, and accountability necessary for carrying out the assigned duties. In the absence of the Medical Director and Administrator the individual is charged with carrying out the resident care policies established by the facility.
