Description

Provide escort services and transportation to residents for medical appointments utilizing the ATA bus, van or personal vehicle. Complete trip logs for accurate documentation of services rendered and for billing purposes. Must submit logs daily.

Tasks

  • Schedule medical transportation and maintain calendars at Care Stations.
  • Assist residents to and from medical provider’s office/facility.
  • Assist residents at provider’s office/facility as needed and requested.
  • Maintain resident confidentiality of healthcare information as required by Federal regulations.
  • Obtain progress notes, doctor’s orders from providers and returning them to the nursing staff.
  • Report any resident incidents or change in condition immediately to Nurse Manager.

Qualifications / Education

  • Must be 18 years of age or older.
  • Must possess current driver’s license, clean driving record, vehicle registration and insurance and have access to vehicle during scheduled working hours.
  • Must possess the ability to deal tactfully with personnel, residents, family members, and the general public.
  • Must be able recognize and relate pertinent information concerning resident’s requests or concerns to supervisors.
  • Must be CPR and First Aid certified.
  • Must possess a high level of flexibility in regards to changes in job assignments and scheduling.
