Restorative Services Nurse

Description

Assist therapists in evaluating and identifying restorative nursing needs. Evaluate and implement restorative programs under the direction and supervision of professional therapists. Update care plans as needed and communicate changes with Care Plan Coordinator. Participate in resident care plans as appropriate.Determine short term, measurable, realistic goals reflective of resident's condition and prognosis, and change as needed and reassess at least monthly.

Tasks

  • Set short-term, realistic goals based on condition and reassess monthly; update as needed.
  • Schedule and revise the restorative nursing program as resident needs change.
  • Direct aides and CNAs to maintain function after therapy discharge.
  • Monitor POC documentation and monthly notes for progress or regression.
  • Write quarterly notes for restorative care and monthly notes for Our House residents.

Qualifications / Education

  • Must have completed a State-approved nursing training and competency evaluation program.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies and the general public.
  • Must be able to relate pertinent information concerning resident’s requests or concerns to Nursing Supervisor and therapy department.
  • Must possess a high level of flexibility in regards to changes in job assignments and scheduling.
  • Must possess the ability to determine specific goals, measure progress towards goals, and accurately document results of therapeutic interventions.
