Patient Services Representative

A Patient Services Representative is responsible for welcoming patients upon arrival, promoting the usage of new and emerging consumer-friendly technologies, educating patients on their copayments and financial obligations and collecting payments when applicable, scheduling subsequent appointments within the continuum of care, connecting patients to financial advocacy resources when appropriate, updating patient's demographics and insurance coverage information, and promoting an overall culture of service excellence. A Patient Services Representative displays strong customer service skills that ensure that patients and family members have the desired experience.

Medical Records Specialist

Maintains patient’s electronic health records for Universal Primary Care (Southern Tier Community Health Center Network, Inc. (STCHCN)) to ensure patient’s health information is complete, accurate, up to date, and protected. Responsible to ensure quality, integrity, security, and protection of patients’ health information.

Medical Referrals Coordinator

This position is responsible for working with providers, care team members, other medical clinics, hospitals, and patients to arrange and schedule referral appointments, monitor referral appointment status, and ensure the referral consultation report comes back to our referring provider for collaborative review and quality care.

Outreach Coordinator

This position is responsible for reaching out to patients identified as lost to care or due for preventive care services and link them with the appropriate care.

Care Coordinator

This position is responsible for identifying, coordinating, and assisting patients with social determinates of health needs to improve patient health outcomes.

Medical Assistant

The medical assistant is responsible for administrative and basic clinical duties under the direct supervision of a registered nurse, licensed physician, or other licensed health care provider with legal authority.

Care Coordination Supervisor

Responsible for overseeing the daily operations of the department and staff.  Provides education and self-management tools for patients identified as eligible for transitional care management or chronic disease management services.  

Prior Authorization Specialist

Responsible for obtaining pre-certifications and pre-authorizations for procedures, tests, medications, and scheduling appointments for patients. Responsibility includes working cohesively and collaboratively with providers and clinical teams to ensure patients prior authorizations are completed in a timely manner and scheduled in a convenient manner for the patients.

Licensed Practical Nurse (LPN)

This position is responsible for answering and reviewing telephone messages, contacting patients to gather additional information, and supporting providers in the development of treatment plans.  This role includes performing tasks and responsibilities within the framework of case finding, health teaching, health counseling, and the provision of supportive and restorative care under the direction of a registered professional nurse, licensed physician, or other licensed health care provider legally authorized to do so.  This position may also be assigned to assist with the training and orientation of new employees, and to work 1:1 with providers on an as needed basis.

Nursing Supervisor

Responsible for overseeing the daily operations of the department and staff.  Provides education and self-management tools for patients identified as eligible for transitional care management or chronic disease management services.  

Registered Nurse (RN) Care Manager

This position is responsible to diagnose and treat human responses to actual or potential health problems through services which include case finding, health teaching, health counseling and provision of care supportive to or restorative of life and well-being and executing medical regimens prescribed by a licensed physician, or other licensed health care provider legally authorized to do so.  This position may provide direction and guidance to LPN’s, Medical Assistants, office staff and others as appropriate.  This position may provide training and orientation to new staff.

Director of Clinical Services

This position is responsible for working collaboratively with providers and team members to ensure efficient, quality care delivery consistent with the mission of Universal Primary Care.  This position is responsible for the duties associated with an acting Director of Nursing within the organization.  This position directly oversees and manages the patient services team, Medical Referrals Coordinators, Prior Authorization Specialist, Registered Nurses, Care Coordination Supervisor, and the Nursing Supervisor.  

Nurse Practitioner (NP)/Physician Assistant (PA)

Under the general supervision of the lead physician and in accordance with standardized procedures, this position is responsible for providing primary care to ambulatory patients through regular health assessments and the management of common acute and chronic stable conditions. Obtains a health history and performs a physical assessment, identifies problems, and formulates plans that include treatment, health education and counseling. Collaborates with other health care professionals in assessing and managing health care problems and maintaining continuity of care.
