Description

This position is responsible for answering and reviewing telephone messages, contacting patients to gather additional information, and supporting providers in the development of treatment plans.  This role includes performing tasks and responsibilities within the framework of case finding, health teaching, health counseling, and the provision of supportive and restorative care under the direction of a registered professional nurse, licensed physician, or other licensed health care provider legally authorized to do so.  This position may also be assigned to assist with the training and orientation of new employees, and to work 1:1 with providers on an as needed basis.

Tasks

  • Answer telephone calls/messages and gather additional information that the provider may need. This may include calling patient to get more information or gathering results, obtaining PMP’s, obtaining lab/DI results and other reports.
  • Demonstrates accountability for the provision of nursing care
  • Collects data and communicates patient information in order to optimize patient outcomes
  • Respects cultural customs and beliefs
  • Assists in determining the health status and health needs of the patient and assists in the formulation of goals/outcomes
  • Demonstrates the knowledge and skill necessary to provide care appropriate to the age and developmental level of the patient
  • Assists in the coordination of human and material resources for the provision of care to assigned patients
  • Participates with other members of the health care team for patient care delivery
  • Demonstrates professional growth as evidenced by a minimum of ten (10) hours of related education in-services annually
  • Responsible for working with providers to best efficiently meet their needs and our patients’ needs
  • Provides stocking, storage, cleaning, communication nurse duties and others as assigned
  • Responsible for NY and PA Rx EMR paper
  • Ensures safety of all medicines and medical supplies maintained at the practice site
  • Supports and assists Director of Nursing in administration of all immunizations, including but not limited to, VFC (Vaccines For Children) program. Monitors and reports to New York State, Director of Nursing and to other staff as needed and required.  Responsible for inventory at all sites in cooperation with clinical site coordinators.
  • Demonstrates professional responsibility and accountability in implementing the nursing process.
  • Utilizes equipment according to company polices/manufacturers standards.
  • Manages and communicates patient information appropriately.
  • Demonstrates the knowledge and skill necessary to provide care appropriate to the age and developmental level of the patient.
  • Respects cultural customs and beliefs.
  • Collaborates with other members of the health care team in order to optimize patient outcomes.
  • Coordinates the use of human and material resources for the provision of care to assigned patients.
  • Identifies and provides the patient and family with appropriate information regarding assessed educational needs.  Assumes responsibility for a role in the education of students, staff and community.
  • Demonstrates professional growth as evidenced by a minimum of ten (10) hours of related educational in-services annually with five (5) hours pertaining to the primary unit worked.
  • Assists in data collection for Quality Assurance Plan.
  • Completes pre-visit planning/chart preparation as needed.
  • Participates in team huddle daily.
  • Provides patient education as needed. 

Qualifications / Education

  • Graduate of an accredited school of Practical Nursing
  • Current, unrestricted Pennsylvania LPN license required, NY state licensure in addition is preferred
  • Successful completion of BLS and Infection Control training required
  • Previous patient care experience in the identified area preferred
