Description

This position is responsible for reaching out to patients identified as lost to care or due for preventive care services and link them with the appropriate care.

Tasks

  • Works in a team dynamic and works closely with supervisor, billing, health information technology, and the quality care management team to identify lost to care patients though insurance rosters. 
  • Works with the same interdisciplinary team members to identify patients due for preventative care screenings though insurance care gaps and quality fallout lists.
  • Manages insurance rosters by reaching out to patients to schedule appointments for those patients identified as lost to care.
  • Manages care gaps and quality rosters reaching out to patients to follow up on screening status and coordinates screening if necessary. 
  • Works with LPN Care Coordinator or providers to request orders for preventative screenings. 
  • Reaches out to patients identified as potentially eligible for sliding fee services and sends out applications to patients. 
  • Reaches out to patients whose sliding fee scale is due to expire and provides them with an updated application. 
  • Works closely with navigators to manage patient population based on program requirements and quality patient care needs. 
  • Maintains appropriate documentation and messaging in the electronic medical record when completing outreach efforts and patient care items.  

Qualifications / Education

  • High School Diploma required.
  • Associates degree in a business or healthcare related field is preferred
  • One (1) to two (2) years of medical office, social work, or clinical practice experience preferred
  • Must have excellent written and verbal communication skills
  • Must have interpersonal skills, compassion, empathy, and excellent customer service skills
  • Experience with Microsoft Office and eClinicalWorks (EMR) preferred
