Care Coordination Supervisor

Description

Responsible for overseeing the daily operations of the department and staff.  Provides education and self-management tools for patients identified as eligible for transitional care management or chronic disease management services.  

Tasks

  • Oversees daily activities and patient panels of the Care Coordination team
  • Schedules transition of care appointments for patients transitioning from inpatient setting to home/community settings
  • Ensures TCM first contact following impatient discharge and follow up to ensure appointment was kept and patient was not readmitted
  • Ensures care management documentation is completed accurately for patients meeting Hep C and MAT eligibility requirements
  • Provides education to patients based on Chronic Care Management eligibility and needs assessment
  • Maintains care plans and other appropriate documentation in electronic medical record
  • Works with HIM Manager to complete monthly reporting
  • Participates in quality assurance projects
  • Provides project management support for UPC initiatives
  • Establishes and maintains relationships with community agencies
  • Works with community agencies to provide community-based outreach and education programs to improve community health outcomes
  • Incorporates EHR and quality initiatives/reimbursements to daily workflow while ensuring linkages to care for patients are available at all sites
  • Works closely with staff on initiating VBP/payor reimbursement plans and workflows for increased reimbursement incentives.
  • Works with teams to develop and implement workflows, documentation requirements, and care plans to address patient and population health initiatives. 
  • Supervisor Responsibilities:
    • Accomplishes department objectives by supervising employees and organizing and monitoring work processes.

    • Accomplishes employee job results by coaching, counseling, and disciplining employees.

    • Maintains a safe and healthy work environment by establishing and enforcing organization standards and adhering to legal regulations.  
    • Develops schedules, assigning and monitoring work, gathers resources, and resolves problems.

Qualifications / Education

EDUCATION/EXPERIENCE
  • High School Diploma or equivalency 
  • 1-2 years medical office experience required
  • 1-2 years of customer service or face to face patient experience required
  • 1-2 years care coordination experience preferred
  • Excellent problem-solving experience required
  • Excellent time-management experience required
  • Knowledge of billing principals and familiarity with ICD-10 coding preferred

  • Basic Microsoft Office suite knowledge
  • Verbal and written communication skills
  • Excellent organizational skills
