Nursing Supervisor

Description

Responsible for overseeing the daily operations of the department and staff.  Provides education and self-management tools for patients identified as eligible for transitional care management or chronic disease management services.  

Tasks

  • Oversees daily activities of the nursing team
  • Establishes a compassionate environment by providing support to patients and families
  • Identifies ways to improve communications and relationships among clinical staff at all UPC sites which may include a consistent schedule of visits to all off-site locations 
  • Assumes responsibility for personal and professional growth and development. Continually evaluates and identifies educational needs and develops training programs to address needs. 
  • Collaborates with the Director of Nursing to implement patient care.
  • Collaborates with the Director of Nursing to analyze and evaluate nursing workflows, procedures, and processes to ensure department effectiveness and efficiencies.  
  • Collaborates with the Director of Nursing to develop, update, maintain, and ensure nursing competencies are in place and that nurses are competent to complete nursing duties adequately and safely. 
  • Completes patient care requirements by scheduling and assigning nursing and staff and following up on work results.
  • Provides information to patients and team by answering questions and requests.
  • Collaborates with providers and administrative team to improve the quality of services and to resolve identified problems at multiple sites
  • Completes employee performance evaluations and clinical competencies, including identification of disciplinary problems, counseling, and implementation of performance improvement plans. 
  • Interacts courteously with patients, family members, employees, colleagues, subordinates, providers, vendors, and community representatives
  • Supervisor Responsibilities:
    • Accomplishes department objectives by supervising employees and organizing and monitoring work processes.
    • Maintains employee recruiting, selecting, orienting and training employees and developing personal growth opportunities.
    • Accomplishes employee job results by coaching, counseling, and disciplining employees.
    • Conducts performance appraisals
    • Conducts training
    • Implements and enforces policy and procedures
    • Maintains a safe and healthy work environment by establishing and enforcing organization standards and adhering to legal regulations.  
    • Develops schedules, assigning and monitoring work, gathers resources, and resolves problems.
  • Performs other duties as assigned.

Qualifications / Education

EDUCATION/EXPERIENCE
  • NYS and PA LPN licensure required
  • NYS and PA RN licensure preferred
  • 1-2 years nursing experience preferred
  • 1-2 years management experience required
  • Excellent problem-solving experience required
  • Excellent time-management experience required

  • Basic Microsoft Office suite knowledge
  • Verbal and written communication skills

  • Excellent clinical skills
