Medical Referrals Coordinator

Description
This position is responsible for working with providers, care team members, other medical clinics, hospitals, and patients to arrange and schedule referral appointments, monitor referral appointment status, and ensure the referral consultation report comes back to our referring provider for collaborative review and quality care.
Tasks
- Maintains patient’s referral records following all referral processes and organizational policies and procedures, and ensures referrals are completed in a timely manner.
- Requests patients’ consult report from referring provider offices and/or hospitals, vendors, other medical facilities as requested.
- Reviews incoming referrals for relevance and attaches the referral to the correct patient’s chart and assigns the referral to the proper healthcare professional for review.
- Maintains patient health information confidentiality, privacy, and security according to HIPAA, and all STCHCN’s healthcare medical record privacy policies.
- Answers and documents incoming patient calls and requests electronically in a timely manner.
- Communicates information clearly, verbally, written, and electronically with other employees and health professionals. Relays referral information and messages in an appropriate and timely manner.
- Works well with STCHCN’s co-workers and team members to ensure new referral processes developed from innovations, initiatives, or IT advancements are utilized and working smoothly for data collection and quality improvement programs. Uses problem solving skills to resolve duty specific tasks.
- Participates in organizational development provided to ensure referral skills are kept up to date in the fast pace changing healthcare industry.
- Keeps an updated record and knowledge of surrounding referral specialists and informs providers of new specialists/services in the area for referrals.
Qualifications / Education
EDUCATION/EXPERIENCE
- High School Diploma or equivalent required
- Associates degree is preferred
- Must posses excellent professionalism, interpersonal skills, attention to detail, customer service skills, written and oral communication skills, listening skills, and organizational skills
- Experience in a medical office setting and medical office tasks preferred
- Experience with Microsoft Office and eClinicalWorks (EMR) preferred
- Experience with medical referrals preferred
