Certified Recovery Specialist

Responsible for providing support and guidance to individuals struggling with addiction issues or co-occurring substance abuse and mental health issues. The position will provide outreach, mentoring, and peer support at all stages of the recovery process. The position includes face to face contact both individually and in groups; assisting individuals in developing a recovery plan and achieving objectives; assisting individuals in selecting services and/or program offerings that will be useful in achieving their recovery goals; offering support and mentoring as needed in the implementation of the recovery plan; and, exploring, securing and utilizing each client's support network and/or community resources.

Part-Time Aide

Responsible for assisting the Rehabilitation Coordinators and Therapists in providing the necessary support and information needed to implement each clients’ treatment plan. The incumbent also assists clients with daily activities, medication monitoring, management of daily tasks and providing transportation when needed.

Rehabilitation Technician

Responsible for providing assistance with activities of daily living and instrumental activities of daily living to clients residing in the Personal Care Home. The incumbent is expected to promote a home-like atmosphere and encourage positive social experiences and interactions with and between clients.

Program Specialist

Responsible for assisting staff in providing the necessary support and information needed to implement each clients' treatment plan. The incumbent also assists clients with daily activities, medication monitoring, management of daily tasks and providing transportation when needed. Crisis management and support is also an important function of this position.

Counselor

Responsible for the treatment and care coordination for clients at Gage House. Duties include: level of care assessments, intake/orientation, developing and implementing preliminary and comprehensive treatment plans, as well as providing individual and group counseling to provide clients with the opportunity to work towards treatment goals and explore recovery. The incumbent will also coordinate aftercare services for services after discharge. The incumbent is responsible for the documentation of these services within the timelines designated by the agency and regulatory bodies.

Certified Peer Specialist

Responsible for providing support and guidance to individuals enrolled in the LTSR program. Responsibilities include, but are not limited to, the following: providing orientation to clients in programs and/or services that might be unfamiliar to them, both individually and in groups; assisting individuals in identifying recovery goals; assisting individuals in selecting services and/or program offerings that will be useful in achieving their recovery goals; offering support and mentoring as needed in the implementation of the recovery plan; exploring, securing and utilizing each clients’ support network and/or community resources; medication monitoring; and, crisis intervention. The incumbent is also responsible for facilitating the development of Wellness Recovery Action Plans (WRAP) with program participants to promote the recovery process. The Certified Peer Specialist, along with every member of the LTSR team, must adhere to the fidelity standards, DPW regulations, and the policies and procedures established by the organization.

Mental Health Worker

Responsible for implementing rehabilitation and support interventions as outlined in the treatment plan for clients enrolled in the LTSR program. Responsibilities include, but are not limited to, case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention, individual and group psychosocial education and supportive counseling, and consultation with families. The incumbent is expected to provide recovery-oriented interventions according to each client’s individual treatment plan and maintain professional relationships with other providers, both internal and external to the program. The Mental Health Worker, along with every member of the LTSR team, must adhere to the DPW regulations and the policies and procedures established by the organization.

Mental Health Professional

Provides treatment, rehabilitation, and support services to clients enrolled in the LTSR program. Responsibilities include, but are not limited to, the following: individual and group therapy, case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention, psychosocial education, vocational support and supportive counseling. The incumbent is expected to provide recovery-oriented interventions according to each client’s individual treatment plan and maintain professional relationships with other providers, both internal and external to the program. The MHP, along with every member of the LTSR team, must adhere to the DPW regulations and the policies and procedures established by the organization.

DAOP Manager

Responsible for the clinical management of the Dual Outpatient programs., inclusive of Outpatient/Intensive Outpatient Drug and Alcohol programming, Erie Prison D/A program, Partial Hospitalization program, CROMISA project, and the Certified Recovery Specialist program. The incumbent is responsible for supervision and mentoring of clinical staff who provide therapeutic services to adults with co-occurring mental health and substance abuse disorders, and support staff who contribute to the effective and efficient operation of the program. The incumbent will also provide quality group and individual therapy to clients seeking dual diagnosis services. Other responsibilities: chart monitoring for clinical records, ensuring compliance with all regulatory and licensing requirements; facilitation of continuous quality improvement efforts; and, ongoing program development that promotes recovery for all individuals served.

Receptionist

Responsible for greeting and attending to all clients and visitors of the clinic. Duties include answering and routing all incoming calls and messages, entering all appointments into the computer database, and maintaining and coordinating interaction between staff and clients. This position is responsible for the completion and maintenance of all data required for the purpose of financial billing.

Gage House Manager

Responsible for the clinical management of the 3B and 3C programs. The incumbent is responsible for supervision and mentoring of clinical staff who provide therapeutic services to adults with co-occurring mental health and substance abuse disorders, and support staff who contribute to the effective and efficient operation of the program. The incumbent will also provide quality group and individual therapy to clients seeking dual diagnosis services. Other responsibilities: chart monitoring for clinical records, ensuring compliance with all regulatory and licensing requirements; facilitation of continuous quality improvement efforts; and, ongoing program development that promotes recovery for all individuals served. Perform utilization reviews and community assessments.

Long-Term Structured Residence (LTSR) Manager

Responsible for the clinical and administrative management of the Long Term Structured Residential Treatment Facility (LTSR). Duties include the supervision and mentoring of clinical and direct care professionals who provide treatment and support services to individuals with mental health needs in the LTSR; budget development and monitoring; enforcement and monitoring of regulatory requirements; and direction of overall program operation. The incumbent will maintain a focus on mental health recovery approaches through leadership by example, coaching and the development of programmatic systems that promote interdisciplinary collaboration.

Director of Addiction Treatment Services

Responsible for the clinical and administrative management and integration of programs within the Addiction Treatment Services Division to include: Outpatient/Intensive Outpatient Drug and Alcohol Programming, Erie County Prison D/A Program, Partial Hospitalization programming, and the Certified Recovery Specialist Program. The incumbent is responsible for the daily operation of the Drug and Alcohol Outpatient Clinic and Gage House Program which includes supervision and mentoring of supervisory and clinical staff who provide therapeutic services to adults with co-occurring mental health and substance abuse disorders as well as support staff who contribute to the effective and efficient operation of the programs. The incumbent will also provide clinical coverage as needed, mentoring, and program development. Other responsibilities include budget development, implementation and monitoring; development and enforcement of policies and procedures; ensuring compliance with all regulatory and licensing requirements; facilitation of continuous quality improvement efforts; and, ongoing program development that promotes recovery for all individuals served. The incumbent collaborates regularly with the Administrative Director of Rehabilitation Services, Administrative Director of Residential Services, Administrative Director of Treatment Services, as well as the Executive Leadership Team.

Senior Director of Residential and Addiction Services

Responsible for the management and integration of the programs within the Residential Division which include: Residential Treatment Facility for Adults (RTFA), Long Term Structured Residential (LTSR), Personal Care Home (PCH) and the GAGE House. Responsibilities include, but are not limited to, the following: supervision and mentoring of program directors and supervisors; monitoring internal systems to ensure continuity of care, clinical cohesion and collaboration, communication and information sharing, implementation of best practices, and overall quality of care. This position is also responsible for the clinical and administrative management and integration of programs within the Addiction Treatment Services Division to include: Outpatient/Intensive Outpatient Drug and Alcohol Programming, Erie County Prison D/A Program, Partial Hospitalization programming, and the Certified Recovery Specialist Program. The incumbent is responsible for the daily operation of the Drug and Alcohol Outpatient Clinic and Gage House Program which includes supervision and mentoring of supervisory and clinical staff who provide therapeutic services to adults with co-occurring mental health and substance abuse disorders as well as support staff who contribute to the effective and efficient operation of the programs. This position will also act as the Facility Director for Gage House and the Project Director for SBH DDAP project. The incumbent collaborates regularly with the Administrative Director of Rehabilitation Services and Administrative Director of Treatment Services and is also an active participant on the Executive Leadership Teams.
