Description

Responsible for the treatment and care coordination for clients at Gage House. Duties include: level of care assessments, intake/orientation, developing and implementing preliminary and comprehensive treatment plans, as well as providing individual and group counseling to provide clients with the opportunity to work towards treatment goals and explore recovery. The incumbent will also coordinate aftercare services for services after discharge. The incumbent is responsible for the documentation of these services within the timelines designated by the agency and regulatory bodies.

Tasks

  • Assists clients in choosing treatment strategies aligned with individual goals using best practices.
  • Conducts BDAP-approved biopsychosocial intakes; administers Addiction Severity Index; monitors status.
  • Facilitates counseling groups and individual sessions, tailoring approaches to client needs and recovery.
  • Consistently meets 75% productivity standard (5.5 billing hours per 7.5-hour scheduled workday).
  • Maintains accurate, timely documentation; supports referrals, discharges, and manages no-show clients.

Qualifications / Education

  • Bachelor's level degree in related field or equivalent education and experience as determined by agency standards and guidelines of position.
  • Must have valid Pennsylvania licensure or Pennsylvania Certification Board credentials, including LPC, LCSW, LSW, CAAC, CADC, CAADC, or CSS or be able to acquire license or credentials within two (2) years of hire
