Description
Provides treatment, rehabilitation, and support services to clients enrolled in the LTSR program. Responsibilities include, but are not limited to, the following: individual and group therapy, case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention, psychosocial education, vocational support and supportive counseling. The incumbent is expected to provide recovery-oriented interventions according to each client’s individual treatment plan and maintain professional relationships with other providers, both internal and external to the program. The MHP, along with every member of the LTSR team, must adhere to the DPW regulations and the policies and procedures established by the organization.
Tasks
- Uses recovery principles and best practices to assist clients in choosing relevant treatment strategies.
- Conducts thorough assessments covering psychiatric, physical, social, and functional client history.
- Provides individual/group therapy and skills building per treatment plans and therapeutic milieu.
- Coordinates and monitors treatment plans; develops, revises goals with clients and treatment team.
- Participates actively in daily shift and treatment team meetings to ensure coordinated care.
- Builds and maintains relationships with providers, families, and community for service continuity.
- Manages crises professionally; accesses crisis services as needed; documents all client interactions.
Qualifications / Education
- Master's level degree or higher in generally recognized clinical discipline and one (1) year of mental health clinical experience
- Registered, licensed or certified if profession is governed by such a board in the Commonwealth
