Rehabilitation Technician
Responsible for providing assistance with activities of daily living and instrumental activities of daily living to clients residing in the Personal Care Home. The incumbent is expected to promote a home-like atmosphere and encourage positive social experiences and interactions with and between clients.
Certified Nursing Assistant
Responsible for providing assistance with activities of daily living to clients residing in the Personal Care Home. In addition to assisting with instrumental activities of daily living. The incumbent is expected to focus on medical concerns and wellness, promote a home-like atmosphere and encourage positive social experiences and interactions with and between clients. This position is primarily site-based.
Clinical Care Specialist
Responsible for the assessment, planning and coordination of services for residents of the Personal Care Home. Duties include the following: conducting resident assessments, developing and implementing comprehensive support plans, coordinating treatment and rehabilitation services as indicated, and providing supportive counseling and skills teaching based on each resident’s individualized plan. The incumbent will co-facilitate multidisciplinary team meetings with the Program Supervisor or Director in order to utilize and incorporate the skills, clinical judgment and input of each member of the team when developing new or modified support plans. The incumbent will maintain a focus on mental health recovery principles and practices in the course of creating and implementing therapeutic interventions. Crisis management and resolution are also important functions of this position. Finally, the Clinical Care Specialist will establish and maintain positive and productive relationships with the clients served and their families as well as internal and external service providers.
Personal Care Home (PCH) Supervisor
Responsible for assisting with the supervision of PCH staff that provide services and coverage with the Personal Care Home. The incumbent assists the PCH Supervisor in ensuring that the PCH operates in compliance with all applicable internal and external policies, procedures and regulations. The Assistant Supervisor is also responsible for the development, implementation and coordination of the tenant Life Enhancement Project which provides opportunities for volunteer, work and social connectedness.
Personal Care Home (PCH) Manager
Responsible for overall clinical and administrative supervision of Personal Care Home operations, as well as continuous improvement of individual client care and treatment services for seniors through innovation and effective leadership. This includes oversight of Geriatric Targeted Case Management specialty team, and oversight of the Home PLUS program. Duties include program design, budget monitoring, facility use, determination of appropriate staffing levels, and supervision of program supervisors and specialty staff. The incumbent ensures compliance with all applicable policies, procedures and regulations, both internal and external. The incumbent is responsible for the administration, management and regulatory adherence of the Personal Care Home, including the health, safety and well being of its residents, in addition to the other relevant senior programs. Further, the incumbent will maintain a focus on mental health recovery approaches through leadership by example, coaching and the development of programmatic systems that promote interdisciplinary collaboration.
