Certified Nursing Assistant

Responsible for providing assistance with activities of daily living to clients residing in the Personal Care Home. In addition to assisting with instrumental activities of daily living. The incumbent is expected to focus on medical concerns and wellness, promote a home-like atmosphere and encourage positive social experiences and interactions with and between clients. This position is primarily site-based.

• Assists clients with self-care tasks respectfully; educates on hygiene if assistance is refused.
• Supports clients with instrumental daily activities like shopping, laundry, and appointments.
• Recognizes and intervenes for clients showing physical illness or mental decompensation symptoms.
• Reports and documents abnormal signs and symptoms, including skin issues like bruising.
• Provides a safe environment; identifies, intervenes, and documents crisis situations promptly.
• Accompanies clients to medical procedures, ensuring support and comfort throughout.
• Communicates client health changes accurately to appropriate team members for follow-up.

• Bachelor's degree in a related field and at last 12 months experience as full-time Rehab Tech 1 and/or Rehab Tech 2 within the agency; or
• Associate's degree with at least 3 years of full-time direct care experience within the agency; or
• High school diploma with at least 4 years experience as Rehab Tech 1 and/or Rehab Tech 2 within the agency; or
• Bachelor's degree in a related field with 18 months of full-time (or equivalent) experience from outside the agency; or
• Associate's degree in a related field with at least 30 months of related experience from outside the agency
• Current CNA certification.
