Blended Case Manager

Responsible for assisting individuals in obtaining mental health support services by creation, coordination and implementation of goal plans which incorporate recovery principles as well as resource coordination needs. The incumbent in this position identifies, links, monitors, serves as a resource and supports the activities which assist individuals and their families within the mental health service system. Crisis management and resolution are also important functions in this position. The incumbent must maintain a professional relationship with internal staff and community resource providers.

Blended Case Management Manager

Responsible for the clinical and administrative management of the Blended Case Management program. The incumbent will provide supervision and mentoring to a team of clinical supervisors who, in turn, provide supervision and guidance to teams of Blended Case Managers who work directly with individuals with mental health issues that require resource coordination and support. The incumbent is also responsible for monitoring assigned budget lines and active participation in budget development, monitoring of intake and discharge processes, development and enforcement of policies and procedures, ensuring compliance with all regulatory and licensing requirements, and facilitation of continuous quality improvement efforts. Further, the incumbent will maintain a focus on mental health recovery approaches through leadership by example, coaching and the development of programmatic systems that promote interdisciplinary collaboration.

Courier

Provide internal mail delivery to all Stairways locations and to local community entities as needed. Provide consistent and excellent customer service. Ensure company vehicle is ready for use. Perform administrative tasks as requested. Act as backup for reception position as needed.

Receptionist

Responsible for greeting and attending to all clients and visitors of the clinic. Duties include answering and routing all incoming calls and messages, entering all appointments into the computer database, and maintaining and coordinating interaction between staff and clients. This position is responsible for the completion and maintenance of all data required for the purpose of financial billing.

BCM Supervisor

Responsible for the clinical management, including some administrative oversight, of the blended case management program in accordance with agency policies and philosophy as well as regulatory standards and contractual guidelines. Duties include the hiring, training, supervision and mentoring of a team of direct care professionals who provide support services to individuals with mental health needs. The incumbent will maintain a focus on mental health recovery approaches through leadership by example, coaching and the development of programmatic systems that promote interdisciplinary collaboration.
