Description

Responsible for the clinical management, including some administrative oversight, of the blended case management program in accordance with agency policies and philosophy as well as regulatory standards and contractual guidelines. Duties include the hiring, training, supervision and mentoring of a team of direct care professionals who provide support services to individuals with mental health needs. The incumbent will maintain a focus on mental health recovery approaches through leadership by example, coaching and the development of programmatic systems that promote interdisciplinary collaboration.

Tasks

  • Leadership: Possesses and demonstrates confidence, assertiveness and the ability to manage staff and direct program operation according to the organization’s mission, vision and strategic initiatives; ability to accept and successfully complete tasks and projects as assigned; creates and consistently monitors a clear agenda for program and operational development and follows through on plans; models and maintains objectivity and displays a fair and level approach to conflict resolution and problem solving.
  • Supervision: Provides direct supervision to up to nine (9) case managers; ensures that 75% of case managers’ time is spent in direct contact with clients; maintains appropriate documentation of supervision; maintains and ensures a balanced caseload mixture for each case manager; monitors productivity goals of case managers; conducts reviews of all documentation, including assessments, goal plans, case reviews, correspondence and other related documentation; ensures compliance with ICM, RC (Blended) regulations and Medical Assistance billing requirements.
  • Clinical Knowledge & Skill: Demonstrate working knowledge of the mental health system, available resources and utilization of other community providers as warranted; regularly consults with administrators and blended case management program staff to ensure that clients are availed of the highest quality services; utilizes experience, up-to-date knowledge and best practice guidelines to provide sound clinical leadership to staff; able to model and teach staff how to help clients become active participants in their recovery journey.
  • Staff Recruitment & Development: Practices a proactive approach to recruiting so as to ensure the availability and hiring of a diverse and qualified pool of professionals to fill available positions; provides orientation, training, coaching, supervision, discipline and evaluation to ensure the retention of high quality staff and the provision of effective service; ensures that staff are availed of relevant professional development opportunities to enhance the variety and quality of services.
  • Liaison: Establishes and maintains relationships with other providers and funding sources to ensure the effectiveness of services and supports, the most efficient use of resources, and quality care for clients served; maintains positive relationships and communication with families and/or significant others; serves as the organization’s representative in community team meetings and other planning sessions.

Qualifications / Education

  • Masters level degree in social work, psychology, rehabilitation, activity therapies, counseling, or education and 3 years mental health direct care experience; or,
  • Bachelor’s level degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, education or be a registered nurse and 5 years mental health direct care experience, 2 of which shall include supervisory experience
