Description
Responsible for the clinical and administrative management of the Blended Case Management program. The incumbent will provide supervision and mentoring to a team of clinical supervisors who, in turn, provide supervision and guidance to teams of Blended Case Managers who work directly with individuals with mental health issues that require resource coordination and support. The incumbent is also responsible for monitoring assigned budget lines and active participation in budget development, monitoring of intake and discharge processes, development and enforcement of policies and procedures, ensuring compliance with all regulatory and licensing requirements, and facilitation of continuous quality improvement efforts. Further, the incumbent will maintain a focus on mental health recovery approaches through leadership by example, coaching and the development of programmatic systems that promote interdisciplinary collaboration.
Tasks
- Leadership: Possesses and demonstrates confidence, assertiveness and the ability to manage staff and direct program operation according to the organization's mission, vision and strategic initiatives; ability to accept and successfully complete tasks and projects as assigned; creates and consistently monitors a clear agenda for program and operational development and follows through on plans; models and maintains objectivity and displays a fair and level approach to conflict resolution and problem solving.
- Clinical Knowledge & Skill: Demonstrates working knowledge of the mental health system, as well as other available resources and how to utilize community providers as warranted; utilizes experience, up-to-date knowledge and best practice guidelines to provide sound clinical leadership to staff; effectively guides and monitors the intake and discharge processes to ensure that clinically appropriate decisions are made consistently so that individuals receiving services are availed of the highest quality programming; able to model and teach staff how to help clients become active participants in their recovery journey.
- Staff Recruitment & Development: Practices a proactive approach to recruiting so as to ensure the availability and hiring of a diverse and qualified pool of professionals to fill available positions; provides orientation, training, coaching, supervision, discipline and evaluation to ensure the retention of high quality staff and the provision of effective service; ensures that staff are availed of relevant professional development opportunities to enhance the variety and quality of services.
- Liaison: Establishes and maintains relationships with other providers and funding sources to ensure the effectiveness of services and supports, the most efficient use of resources, and quality care for clients served; maintains positive relationships and communication with families and/or significant others; serves as the organization's representative in community team meetings and other planning sessions.
- Contract Management: Demonstrates fiscal responsibility by adhering to budgetary guidelines; ensures that contract components are fulfilled; reports on client and program outcomes, customer satisfaction, and other measures established to monitor quality programming; ensures that service projections and productivity standards are consistently met.
Qualifications / Education
- Masters level degree in sociology, social work, psychology, gerontology, anthropology, nursing, other related social sciences, criminal justice, theology, counseling or education, and three (3) years mental health direct care experience; or,
- Bachelor's level degree in sociology, social work, psychology, gerontology, anthropology, nursing, other related social sciences, criminal justice, theology, counseling or education, and five (5) years mental health direct care experience, 2 of which shall include supervisory experience
