Description

Responsible for providing comprehensive case management services to support consumers in achieving their goals. This includes recruitment, assessment, eligibility determination, goal planning, referrals, and ongoing follow-up. The role requires maintaining accurate documentation, conducting home visits as needed, and ensuring compliance with service rules and regulations. The Case Manager develops strong knowledge of community resources and the local job market to assist consumers in overcoming barriers and securing employment. Additional responsibilities include mentoring consumers, coordinating outreach activities, delivering public presentations, and participating in required trainings and meetings. Travel is an essential component of this position.

Tasks

1.    Responsible for case management including:  recruitment, assessment, eligibility testing, goal planning, referrals, follow-up, and case documentation; home visits may be required.
2.    Develops and maintains comprehensive knowledge of community resources and provides information and referrals.
3.    Encourages and mentors consumers and assist them overcome barriers and obtain applicable services and support necessary to achieve their planned goals; recognizes consumer’s achievements.
4.    Maintains knowledge of the local job market and job openings to assist consumers to obtain and maintain employment.
5.    Ensures consumer and service data is accurately collected, recorded, and reported within required timelines.
6.    Coordinates and conducts public presentations and community outreach.
7.    Learns and consistently applies service rules and regulations; obtains and maintains required training and certifications.

Qualifications / Education

Bachelor’s degree and one year related experience; or an equivalent combination of education and experience.
