Description
Assure that resident care practices and outcomes as well as professional performance at the facility are regularly, thoroughly, and proficiently reviewed and evaluated with a plan of action developed as indicated to ensure that quality care is provided to all residents.
Tasks
- Teach the philosophy, purpose and processes of CQI.
- Assist other departments in developing indicators and corresponding measuring tools for department specific CQI objectives.
- Assist in the establishment of Action Teams and in the development of the team’s focused agenda.
- Assist Action Teams in breaking down individual assignments and tasks for team members.
- Be available as a support and resource person for Action Teams.
- Attend Action Team meetings as necessary to assist in facilitating the team’s agenda.
- Complete monthly tracking of quality of care indicators (such as pressure ulcers, falls, medication errors, bruises, skin tears, behavior incidents, etc.), being observant for trends and common denominators.
- Prepare reports and graphs illustrating the results of the monthly tracking.
- Track facility performance related benchmarks and prepare reports and graphs of findings.
- Review Quality Measure/Quality Indicator reports and evaluate improvement/decline in facility’s performance displayed through comparison with previous reports.
- Prepare reports and graphs to illustrate facility statistics presented on the QM/QI reports.
- Identify undesirable/unsatisfactory outcomes and participate in the development, implementation and evaluation of a plan of action to obtain the needed improvement.
- Assist in the development, implementation, and monitoring of survey deficiency plan of corrections.
- Complete medical record reviews for any resident with a facility acquired pressure ulcer who does not meet criteria for being a t risk for compromised skin integrity in an effort to identify contributing factors.
- Develop and/or complete monitors and audits of resident care/staffing concerns.
Qualifications / Education
- Associate’s Degree is necessary. Bachelor’s degree is preferred.
- Current RN license is required. Employee must be in good standing with the state. Possession of a current Basic Life Support for Health Care Providers CPR certificate is required.
- Sufficient education in order to perform daily tasks with an understanding of the duties involved. Minimum of five (5) years clinical nursing experience, two of which have been in LTC. Knowledge of State/Federal regulations and knowledge of the CQI process.
