Service Coordinator - Residential Services

Description

Responsible for the general oversight of the individuals admission, discharge and is the primary liaison between the agency and the families, placing agencies and other involved providers; is responsible for all clerical needs of the program including maintenance of medical records.

Tasks

  • Ensure all admission intake paperwork is complete, signed, and filed in a timely manner.
  • Ensure medical record documents are scanned and stored accurately in the EMR or network drive.
  • Obtain necessary and sufficient information from individuals referred for placement.
  • Schedule family and significant other visits in accordance with policies and regulations.
  • Serve as a liaison between individuals, the facility, families, placing agency, and the community.

Qualifications / Education

  • High School Diploma with a minimum of 2 years’ experience working in a health care field.
  • Bachelor's degree (B.A.) from four-year College or university preferred.
  • Must have experience in general clerical skills and office management
