Description

Guide patients through their healthcare journey by providing support, education, and ensuring continuity of care.

Tasks

  • Assist patients in scheduling appointments, tests, and follow-up visits.
  • Educate patients about their conditions, treatment options, medication regimens, and preventive care.
  • Follow up with patients to track progress on their treatment plans.
  • Act as a communication link between patients and physicians, specialists, or other healthcare team members.
  • Help facilitate the flow of information and ensure patients understand their care instructions and treatment plans.

Qualifications / Education

  • High School diploma or equivalent
  • Current LPN Licensure in the State of Pennsylvania required
  • Understanding of Payor Quality guidelines, MIPS initiatives and Pay for Performance programs preferred
  • At least 2 years of experience in physician practice setting required
  • Licensed in the Commonwealth of Pennsylvania
  • Excellent verbal communication skills
  • Knowledge in regulatory requirement of third party payors such as Medicare, Medicaid, and Blue Shield
