Description

Responsible for assessment and evaluation of the diabetes patients needs and formulate a respective care-plan. Educate diabetes patients, both inpatient and outpatient, on self-management techniques. Provide up-to-date information on diabetes education to entire nursing staff.

Tasks

  • Conduct assessments of patients’ diabetes knowledge, lifestyle, and medical history to develop personalized education plans.
  • Teach patients and their families about diabetes, including monitoring, insulin administration, diet, exercise, and complications.
  • Work with patients to track their blood glucose levels, medication adherence, and lifestyle changes.
  • Communicate with the broader healthcare team to ensure coordinated care and provide updates on patient progress.

Qualifications / Education

  • Registered professional as listed under the required professions by the national certification board for diabetes educators, preferably a registered nurse licensed in the commonwealth of PA required
  • Five years experience in clinical nursing with ability to demonstrate knowledge of diabetes management
  • Certified diabetes educator credentials
