Nurse LPN

Responsible for supporting the Registered Nurse with assessments of client physical and mental health needs, planning all medical interventions for inclusion in the treatment plan, facilitating health / wellness education groups, medication monitoring, advocacy as appropriate, and the overall surveillance and maintenance of the health needs of staff and clients. The incumbent will also be responsible for maintaining appropriate medication storage/security, including up-to-date MAR, as well as written and verbal communication with other staff regarding clients' medical conditions and medication needs.

• Coordinates medication monitoring with RN; ensures accurate, error-free MAR documentation.
• Conducts detailed assessments for medication referrals and enrollment decisions.
• Coordinates services between psychiatrist, pharmacy, medical staff, and program team.
• Uses nursing and recovery principles to perform thorough client intake and ongoing assessments.
• Models and teaches clients to actively participate in their recovery and medication management.
• Facilitates medication education groups and provides individual client medication support.
• Participates in professional development and maintains current Licensed Practical Nurse status.

• Current licensure as Licensed Practical Nurse by State of Pennsylvania; experience working with individuals with mental illness, developmental disabilities, and/or substance abuse disorders preferred
