Description

Responsible for providing psychiatric rehabilitation, mobile medication monitoring and support services to clients in the CROMISA program in need of skills training, symptom management, and resource coordination in the living, learning, working, and socialization areas. The incumbent will develop recovery plans collaboratively with the client and the CROMISA treatment team, provide recovery interventions according to each client's recovery plan and provide input to the CROMISA treatment team regarding the client's progress and continued need for such services.

Tasks

  • Utilizes experience, up-to-date knowledge of psychiatric rehabilitation principles, recovery-oriented approaches and drug and alcohol supports to provide sound clinical services; demonstrates ability to build rapport and show positive regard for clients; effectively develops and follows recovery plan and focuses on empowerment while supporting and/or educating clients to achieve their personal goals; able to model and teach clients how to become active participants in recovery process while demonstrating adherence to both professional and legal boundaries.
  • Demonstrates error-free performance related to med monitoring duties, including participation on med run, client education toward self-management of meds, and MAR documentation; provides support to clients prior to and during psychiatric/medical appointments and completes all follow up tasks as necessary; proactively coordinates with clients and other program staff regarding medication management needs.
  • Demonstrates ability to provide skills training and teaching to clients re-integrating into the community using a variety of methods based on individual client needs and goals; able to identify areas where skills teaching is needed and develop strategies and interventions to facilitate training.
  • Effectively assists clients in accessing and utilizing natural resources to include drug and alcohol services, housing resources, use of public transportation, accessing local food banks and discount stores, budgeting money to meet individual needs and living expenses, accessing welfare/social security or other needed benefits, and linking clients with vocational and educational training programs and services.
  • Liaison: Establishes and maintains relationships with CROMISA partners and other providers to ensure continuity of care, resource coordination and effectiveness of services and supports; maintains positive relationships and communication with families and/or significant others; serves as the organization's representative while participating in CROMISA treatment team meetings.

Qualifications / Education

  • Bachelor's degree in a mental health related area; OR an associates degree in a related field with 2 years full time (or equivalent) related experience; OR
  • High school diploma (or equivalent) with 18 months full time experience as a Rehabilitation Technician 1 within the agency, 3 years experience if from outside the agency.
