Description
Responsible for the coordination of person-centered services for individuals within the Community Participation Support program (2390).
Tasks
- Participate in the development and review of the ISP, including updates and revisions of the ISP and attend the ISP meetings; fulfill the role of plan lead when applicable
- Encourage appropriate participant movement through the continuum of employment services with an emphasis on maximizing employment capabilities
- Assure the ISP is implemented as written
- Work with Program Management to coordinate the training of direct support professionals in the content of health and safety needs relevant to each participant
- Acquire, maintain and secure all participant records in line with all regulatory requirements
- Facilitates the admission and discharge processes in line with regulatory and Agency expectations
- Acquire and consider program direct support professional(s) feedback for ISP updates
- Participate in ISPs, SIS assessments, IM4Q, and other participant related meetings
- Offer guidance, support, dispute resolution and direction to participants as necessary
- Arrange for and accompany individuals to appointments as necessary
- Conduct tours of the Agency as requested
- Assist with licensing and audits
- Report incidents per program regulations
- Contributes to providing a safe and healthy environment by monitoring conditions and recommending changes
- Promote, maintain and model behavior that demonstrates respect for others and acknowledges the need s and rights of the participants we support
- Uses discretion and independent judgment in handling confidential and sensitive information in connection with Agency business
- Work in cooperation with other community agencies, businesses and providers as needed
- Ensure adherence to confidential, legal, and Agency policies and procedures
- Participate in professional development, staff meetings, and group conferences
- Maintain high professional standards at work with regard to personal behavior and interactions with other staff, participants, families, and outside agencies

Qualifications / Education
- Associate’s degree in a human service field and 5 or more years’ experience in human services.
- Bachelor’s degree and experience in a related field is preferred.
- Possess a strong working knowledge of all regulations pertaining to Community Participation Support services, as well as excellent written, verbal, and organizational skills.
- Maintain a professional attitude and appearance.
- Personal qualifications include the ability to handle multiple tasks/projects simultaneously, deal with changing priorities, interpret programs to the community, attention to detail, leadership, clinical knowledge, the ability to work in a team environment with all levels of the organization and the understanding of individual needs.
- Must have reliable transportation.
