LPN Nurse

Assists with assessment of resident physical and mental health needs and medical interventions for the treatment plan. Activities may include health/wellness groups, medication monitoring, advocacy and communication of health needs and maintenance, medication administration, medication storage, and maintaining medication documentation information.

• Models and fosters an organizational culture of respect, cooperation, and service excellence.
• Assists with assessment of physical and mental health needs and obtaining medical history. ( i.e. physical examination, vision and hearing screening, TB skin test, prostate examination, allergies, etc.)
• Ensures appropriate immunization records are received.
• Communicates any special precautions, protocols, allergies, dietary instructions, or other specific needs.
• Facilitates blood work according to resident needs and defined procedures.

• High school diploma (or GED)
• Completion of an approved Licensed Practical Nurse program to obtain licensure
• 1 year of experience in mental health and physical settings preferred
• Licensed to practice as a Licensed Practical Nurse in the state of Pennsylvania
