Description

Assists with assessment of resident physical and mental health needs and medical interventions for the treatment plan. Activities may include health/wellness groups, medication monitoring, advocacy and communication of health needs and maintenance, medication administration, medication storage, and maintaining medication documentation information.

Tasks

  • Assist with physical and mental health assessments and medical history collection.
  • Ensure immunization records are complete and received.
  • Communicate special precautions, allergies, diets, and other resident needs.
  • Facilitate blood work per resident needs and established procedures.

Qualifications / Education

  • High school diploma or equivalent.
  • Completion of an approved Licensed Practical Nurse program to obtain licensure
  • 1 year of experience in mental health and physical settings preferred.
  • Licensed to practice as a Licensed Practical Nurse in the state of Pennsylvania.
