Description

Responsible for providing recovery oriented peer support by assisting individuals with identifying Personal Medicine/Wellness Tools, writing Power Statements and assisting consumers with completing the Common Ground Health Reports.

Tasks

  • Assist consumers in skill-building and independence using personal recovery stories.
  • Follow process: open consumer, assess, create goals, document progress, collaborate, close.
  • Train consumers on goal plan components; plan schedule to meet their needs.
  • Attend appointments and complete paperwork timely.
  • Develop therapeutic, professional relationships; apply recovery and strength-based approaches.

Qualifications / Education

  • High School diploma or GED Self- Identified Peer with personal Mental Health experience.
  • Must be able to obtain agency-required clearances for criminal record check, child abuse, and working with children.
  • Must have a lived mental health experience.
