Description
Responsible for providing recovery oriented peer support by assisting individuals with identifying Personal Medicine/Wellness Tools, writing Power Statements and assisting consumers with completing the Common Ground Health Reports.
Tasks
- Assist consumers in skill-building and independence using personal recovery stories.
- Follow process: open consumer, assess, create goals, document progress, collaborate, close.
- Train consumers on goal plan components; plan schedule to meet their needs.
- Attend appointments and complete paperwork timely.
- Develop therapeutic, professional relationships; apply recovery and strength-based approaches.
Qualifications / Education
- High School diploma or GED Self- Identified Peer with personal Mental Health experience.
- Must be able to obtain agency-required clearances for criminal record check, child abuse, and working with children.
- Must have a lived mental health experience.
