Blended Case Manager (BCM) Supervisor

Description

Responsible for the day to day operations of the Blended Case Management (BCM) program. This program provides monitoring, linking and coordination of services to individuals with a behavioral health diagnosis, in Elk and Cameron Counties. Responsible for training and supervising staff, collaborating with internal and external stakeholders, monitoring financial performance and preparing for all licensing and fraud, waste, and abuse audits. Ensures that the services delivered meet agency and regulatory standards. Also has direct oversight of the Wellness Nurse, the Behavioral Health Home Plus Program (BHHP) as well as the Representative Payee Program for identified beneficiaries.

Tasks

  • Monitors financial performance of the program.
  • Monitors case manager productivity and completes Performance Improvement Plans as needed.
  • Actively seeks referrals and markets programs to maintain an adequate census.
  • Ensures case management plans are recovery-oriented with specified consumer outcomes.
  • Complies with Mental Health Bulletin and Community Care Behavioral Health standards.
  • Monitors regulatory changes affecting the program or industry.

Qualifications / Education

  • Master's Degree in Human services field and 3 years mental health direct care experience
  • Bachelor's degree in Human services field or a registered nurse and 5 years mental health direct care experience, 2 of which shall include supervisory experience.
  • Bachelor's degree in nursing and 3 years mental health direct care experience.
