Registered Nurse, Assessment Coordinator

Coordinate the nursing care for our residents, participating in education to residents and families, and supporting the team.  We provide round-the-clock coverage and offer personalized attention to each resident in our home-like atmosphere.  

• Conduct comprehensive assessments to identify resident needs and goals.
• Develop and regularly update individualized care plans.
• Ensure compliance with all relevant regulations and standards.
• Coordinate and lead interdisciplinary team meetings for care planning.
• Monitor and support quality improvement initiatives and outcomes.
• Document assessments, plans, and progress accurately and timely.
• Communicate effectively with staff, residents, and families.

• Have a current PA RN License required
• Have current CPR or BLS Certification preferred
• Have 1-2 yrs experience (preferred)
• Have Long Term Care/Skilled Nursing Facility experience (preferred)
• Have previous medical systems computer experience (preferred)
