Registered Nurse (RN) - Long Term Care

Description

Responsible for delivering care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacting with patients, significant others, and other health care team members while maintaining standards of professional nursing.  

Tasks

  • Perform patient assessments to identify health needs and changes in condition.
  • Develop and update individualized care plans based on assessment findings.
  • Administer medications safely and monitor for side effects or reactions.
  • Educate patients and families on treatments, medications, and self-care.
  • Accurately document patient care, observations, and outcomes.
  • Maintain up-to-date medical records in compliance with regulations.
  • Communicate with the care team to ensure coordinated patient care.

Qualifications / Education

  • Have a current Pennsylvania Registered Nurse License or Temporary Practice Permit
  • Have Healthcare Provider CPR within 6 months of start date in the position
  • Have ability to work flexible hours including shift, weekends, holidays, etc.
  • Have ability to pass Criminal Background Check
