Description

Provide nutrition care through the Nutrition Care Process, by nutrition screening, data collection, and implementation of the Dietitian’s nutrition care plan. Supports food services in the provision of quality meals that are nutritionally adequate and therapeutically accurate. Ensures resident satisfaction, quality care, and regulatory agency compliance are achieved through the safe and efficient use of resources. Promotes a culture of wellness for residents.

Tasks

  • Conduct nutritional assessments to evaluate dietary needs.
  • Create meal plans that meet individual nutrition requirements.
  • Educate and counsel patients on healthy eating habits.
  • Monitor patient progress and adjust nutrition plans as needed.
  • Document assessments, plans, and outcomes accurately.
  • Collaborate with healthcare teams to support patient care.
  • Provide nutrition resources tailored to patient conditions.

Qualifications / Education

  • Associates or 4 year degree in Nutrition and Dietetics
  • Current registration as NDTR (Nutrition and Dietetics Technician Registered) by CDR (within 90 days of hire)
  • Previous experience in long term care preferred
  • Microsoft Office Skills
  • Active member of the Academy of Nutrition and Dietetics
