Description
Responsible for the overall provision and coordination of person-centered services for individuals supported at The Blaisdell Center.
Tasks
- Lead and update ISP/Care Plans; ensure proper implementation and communication to staff.
- Supervise Blaisdell Center DSPs and Activities staff; support their growth and performance.
- Coordinate specialized training and maintain regulatory knowledge of ODP, DOH, DOA, etc.
- Maintain participant records and support admissions, discharges, and audits.
- Monitor health and safety, resolve concerns, and ensure a respectful environment.
- Participate in ISP, SIS, IM4Q, Human Rights, and other required meetings.
- Ensure policy compliance, report incidents, and model high professional standards.
Qualifications / Education
- Bachelor’s degree in a human service field and 2 or more years’ experience working with individuals with disabilities, aging adults, and adults with functional impairment.
- Master’s degree and experience in a leadership role is preferred.
- Possess a strong working knowledge of all regulations pertaining to services provided at The Blaisdell Center, as well as excellent written, verbal, and organizational skills.
- Maintain a professional attitude and appearance.
- Personal qualifications include the ability to handle multiple tasks/projects simultaneously, deal with changing priorities, to interpret programs to the community, attention to detail, leadership, clinical knowledge, supervisor abilities, the ability to work in a team environment with all levels of the organization and the understanding of individual needs.
- Must have reliable transportation.
