Description

Responsible for the coordination of person-centered services for individuals within the Community Participation Support program (2390).

Tasks

  • Lead or participate in ISP development, updates, and related planning meetings.
  • Ensure ISPs are implemented as written and support participant employment progression.
  • Train and support DSPs on participant-specific health and safety needs.
  • Maintain accurate participant records and assist with admissions and discharges.
  • Provide participant guidance, attend appointments, and resolve concerns as needed.
  • Support licensing, audits, incident reporting, and create safe environments.
  • Uphold agency policies, professionalism, and promote participant rights and respect.

Qualifications / Education

  • Associate’s degree in a human service field and 5 or more years’ experience in human services.
  • Bachelor’s degree and experience in a related field is preferred.
  • Possess a strong working knowledge of all regulations pertaining to Community Participation Support services, as well as excellent written, verbal, and organizational skills.
  • Maintain a professional attitude and appearance.
  • Personal qualifications include the ability to handle multiple tasks/projects simultaneously, deal with changing priorities, interpret programs to the community, attention to detail, leadership, clinical knowledge, the ability to work in a team environment with all levels of the organization and the understanding of individual needs.
  • Must have reliable transportation.
