Description
Work directly with mental health consumers by assisting persons with functional disabilities resulting from mental illness to develop, enhance, and/or retain psychiatric stability, social competencies, personal adjustment, and/or independent living skills. May perform supervisory functions as delegated by the director. Discussing approaches to assist individuals in goal attainment.
Tasks
- Coordinates med monitoring: tracks meds, conducts intakes, and enrolls clients per procedures.
- Uses rehab principles to build, update, and guide individualized med management plans.
- Performs med runs, educates clients, completes MARs, and supports appointments and follow-ups.
- Partners with staff and clients to meet all medication-related needs proactively and efficiently.
- Maintains strong relationships with families and providers for continuity and collaboration.
- Applies recovery-focused, evidence-based methods to teach med self-management to clients.
- Provides tailored skills training individually or in groups, based on client goals and needs.
Qualifications / Education
- Bachelor's level degree in a mental health related field with at least two years of related full-time (or equivalent) experience; or Masters level degree in a mental health related field with at least one year of related full-time (or equivalent) experience.
- Bachelor's level degree in a mental health related field with at least three years of related full-time (or equivalent) with at least 48 months of experience as a Rehabilitation Coordinator 1; or Masters level degree in a mental health related field with at least 24 months of experience as a Rehabilitation Coordinator 1.
