Rehabilitation Coordinator - Psych Rehab

Responsible for assisting in the monitoring and coordination of referrals, conducting and assigning client intakes, and monitoring discharges to coordinate prompt admissions to program, providing in-home mobile medication monitoring services to clients in need of skills training, symptom management, and resource coordination and conducting authorization calls to the Managed Care Companies. The incumbent must maintain a professional relationship with internal program staff and community resource providers which includes participation in case review/treatment team meetings for individuals being referred to program as well as responsible for coordinating Mobile Medication Monitoring (MMM) services prior any individual's discharge from residential providers and local hospitals.

• Coordinates med monitoring program, tracking meds, conducting intakes, and enrolling clients.
• Uses rehab principles to create and adjust med plans for recovery-oriented service delivery.
• Performs med runs, educates clients, supports appointments, and documents meds without error.
• Collaborates with staff and clients to proactively meet med management needs and follow-ups.
• Builds strong ties with families and providers to ensure continuity and coordination of care.
• Applies recovery-based practices and crisis skills to coach clients in medication self-management.
• Teaches practical skills to clients individually or in groups, based on assessed needs and goals.

• Bachelor's level degree in a mental health related field with at least two years of related full-time (or equivalent) experience; or Masters level degree in a mental health related field with at least one year of related full-time (or equivalent) experience.
• Bachelor's level degree in a mental health related field with at least three years of related full-time (or equivalent) with at least 48 months of experience as a Rehabilitation Coordinator 1; or Masters level degree in a mental health related field with at least 24 months of experience as a Rehabilitation Coordinator 1.
