Description

Responsible for enhancing the abilities of clients to lead self-determined lives by providing the necessary support and information for them to implement their rehabilitation / treatment plans in a collaborative and expeditious manner. The incumbent also assists clients with scheduled activities, medication monitoring, management of daily tasks, providing transportation when needed, group facilitation, intake information gathering and general collaboration with the Rehabilitation Coordinator, Nurse and other staff. Crisis management and support is also an important function of this position.

Tasks

  • Uses rehab and recovery principles to support and empower clients toward goal achievement.
  • Handles all levels of intake/assessment; builds rapport while maintaining professional boundaries.
  • Identifies behavioral changes and provides timely intervention or referral.
  • Facilitates psychoeducational groups on relapse prevention, stress, anger, and relaxation skills.
  • Educates clients on mental illness, substance abuse, medication effects, and interactions.
  • Assists clients in accessing community resources for vocational, financial, and housing needs.
  • Maintains accurate documentation per regulatory and organizational guidelines.

Qualifications / Education

  • High school diploma or equivalent
