Description
Responsible for implementing rehabilitation and support interventions as outlined in the treatment plan for clients enrolled in the LTSR program. Responsibilities include, but are not limited to, case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention, individual and group psychosocial education and supportive counseling, and consultation with families. The incumbent is expected to provide recovery-oriented interventions according to each client’s individual treatment plan and maintain professional relationships with other providers, both internal and external to the program. The Mental Health Worker, along with every member of the LTSR team, must adhere to the DPW regulations and the policies and procedures established by the organization.
Tasks
- Uses recovery-based approaches and best practices to provide clinical services per treatment plans.
- Conducts basic screenings to evaluate client symptoms and progress toward goals.
- Provides individual and group support, focusing on mental health, physical health, and daily living skills.
- Coordinates treatment activities; participates in daily staff and treatment team meetings.
- Builds relationships with providers and families; stays current on community resources and programs.
- Recognizes and manages crises; accesses crisis services with supervisory guidance as needed.
- Maintains accurate documentation of all client and provider interactions per regulatory guidelines.
Qualifications / Education
- Completion of 12 semester hours of college training in a mental health related field; or
- A high school diploma or equivalency and 6 months of formal training in mental health or a related field; and 2 years' experience in direct care of clients with a mental health diagnosis
