Description
Responsible for implementing rehabilitation and support interventions as outlined in the treatment plan for clients enrolled in the ACT program and under the clinical supervision of staff with professional degrees. Responsibilities include, but are not limited to, case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention under the clinical supervision of the Team Leader and Psychiatrist, psychosocial education, vocational support and supportive counseling, and consultation with families. The incumbent is expected to provide recovery-oriented interventions according to each client’s individual treatment plan and maintain professional relationships with other providers, both internal and external to the program. The Mental Health Worker, along with every member of the ACT team, must adhere to the fidelity standards, DPW regulations, and the policies and procedures established by the organization.
Tasks
- Uses best practices to provide clinical services and skills sessions per the treatment plan.
- Conducts screenings to assess symptoms, monitor progress, and support client goals.
- Coordinates treatment plans and manages care for assigned clients and team activities.
- Builds provider/family ties, uses resources, and represents the agency in the community.
- Responds effectively to crises in all settings; provides on-call and backup support as assigned.
- Maintains accurate documentation per internal and regulatory standards.
- Stays informed of program changes and ensures clients access appropriate community supports.
Qualifications / Education
- Bachelor's level degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education; or,
- High school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years experience in public or private human services with 1 year in direct client contact.
