Description
Responsibilities include, but are not limited to, the following: case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention, psychosocial education, vocational support and supportive counseling. The incumbent is expected to provide recovery-oriented interventions according to each client’s individual treatment plan and maintain professional relationships with other providers, both internal and external to the program. The MHP, along with every member of the ACT team, must adhere to the fidelity standards, DPW regulations, and the policies and procedures established by the organization.
Tasks
- Uses best practices to help clients set goals, assess needs, and provide therapy and skill sessions.
- Coordinates care for assigned clients, leads treatment planning, and manages team shift duties.
- Builds provider/family relationships to ensure care continuity and positive communication.
- Responds to crises effectively in person or remotely; provides on-call coverage as assigned.
- Maintains accurate documentation per internal and regulatory guidelines.
- Participates in team meetings and treatment evaluations to improve service outcomes.
- Supports recovery by aligning treatment plans with client needs and community resources.
Qualifications / Education
- Master's level degree & licensure in related field.
