Description
Responsible for the assessment, planning and coordination of services for residents of the Personal Care Home. Duties include the following: conducting resident assessments, developing and implementing comprehensive support plans, coordinating treatment and rehabilitation services as indicated, and providing supportive counseling and skills teaching based on each residentās individualized plan. The incumbent will co-facilitate multidisciplinary team meetings with the Program Supervisor or Director in order to utilize and incorporate the skills, clinical judgment and input of each member of the team when developing new or modified support plans. The incumbent will maintain a focus on mental health recovery principles and practices in the course of creating and implementing therapeutic interventions. Crisis management and resolution are also important functions of this position. Finally, the Clinical Care Specialist will establish and maintain positive and productive relationships with the clients served and their families as well as internal and external service providers.
Tasks
- Applies recovery-oriented interventions and psychiatric rehab principles; models effective team communication.
- Conducts detailed client assessments; develops and updates support plans; co-facilitates team meetings.
- Tracks client participation in therapeutic activities; provides consultation and coordinates resources.
- Facilitates therapeutic and psychoeducational groups; supports mental health stability and skill building.
- Promotes client independence by addressing mental health and symptom management in sessions.
- Maintains collaborative relationships with internal and external providers; ensures continuity of care.
- Represents organization in community and discharge planning meetings; engages families and supports.
Qualifications / Education
- Bachelor's level degree and 2 years' experience or equivalent education and experience, as determined by agency standards and guidelines of position.
