Description
Functions at a professional nursing level using administrative leadership skills, assessment skills and an understanding of the clinical reimbursement system to coordinate the interdisciplinary assessment and care planning process. The Licensed Practical Nurse Assessment Coordinator (LPNAC) ensures that the Resident Assessment Instrument (RAI) is completed in accordance with regulatory and financial requirements for all state and federal agencies, and oversees the development of individual resident care plans through interdisciplinary collaboration. The LPNAC is responsible for accurate and timely transmission of the MDS 3.0 documents to the state agency, and assists the facility staff and ancillary staff through in-service programs to understand the RAI, care planning and reimbursement process. The LPNAC reports to the Administrator and works in collaboration with the Director of Nursing.
Tasks
- Provide skilled nursing care in accordance with care plans.
- Administer medications and monitor patient responses.
- Assist with wound care, IV therapy, and specialized treatments.
- Document patient progress and update medical records.
- Coordinate with healthcare teams for optimal patient outcomes.
- Educate patients and families on care procedures and recovery.
Qualifications / Education
- PA LPN license 1 year of management (or comparable) experience in a long term care setting as an LPN
