Social Worker

Responsible for providing medically related social services to facility residents. Identify resident needs and assure they are addressed by the appropriate discipline. Function as a member of an interdisciplinary team which strives to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.

• Obtain pre-admission paperwork from referral source or applicant.
• Complete all required paperwork during the resident admission process.
• Provide Residents’ Rights and Responsibilities information at admission.
• Confirm presence of Advance Directive; provide forms and info as needed.
• Arrange for needed durable and adaptive medical equipment.

• Must have a Bachelor’s degree in Social Work or in a human services field including, but not limited to, sociology, special education, rehabilitation counseling and psychology.
• One year supervised Social Work experience in a health care setting working directly with individuals.
• Applicant with a college degree other than those listed above may be considered if supervised by a Social Work Consultant.
• Experience in long term care preferred.
• Must exemplify good communication and interpersonal skills.
