Resident Care Coordinator

Description

Oversee the Care Plan process and serve in a support capacity to the Registered Nurse Assessment Coordinator(s).

Tasks

  • Lead Care Plan Conferences with residents, families, staff, and outside providers.
  • Work with the care team to create written care plans with goals, services, and assignments.
  • Ensure care plans include clear objectives and timelines based on MDS assessments.
  • Ensure initial care plans are completed within 21 days of each resident’s admission.
  • Ensure quarterly care plan reviews are completed on schedule.

Qualifications / Education

  • Must have successfully completed a State-approved training program and be currently licensed to practice nursing by the Pennsylvania Department of State, Bureau of Professional and Occupational Affairs.
  • Must possess knowledge of the aging process and possess the ability to evaluate the resident’s age-specific needs.
  • Must have excellent oral and written communication skills and be proficient utilizing a computer.
  • Must possess strong organizational and time-management skills.
  • Must possess a high degree of interpersonal skills.
  • Must possess the ability to make independent decisions.
