Description
Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of CATs and CAAs.
Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans.
Conduct or coordinate the interviewing of each resident for the resident’s assessment. Work with the Interdisciplinary Care Plan Team in developing a comprehensive resident assessment and care plan for each resident and ensure that appropriate health professionals are involved in the assessment and the team is aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties for false certification.
Tasks
- Maintain positive relationships with health pros, physicians, and agencies involved in care planning.
- Accompany attending physicians on rounds as needed.
- Review resident conditions to assess needs or revise admission assessments.
- Work with DON to alert assessment team of new resident admissions.
- Help directors and supervisors schedule assessments; support Care Plan Coordinator.
- Ensure care aligns with each resident’s expressed wishes and preferences.
Qualifications / Education
- Must be currently licensed as a Registered Nurse to practice by the Pennsylvania Department of State, Bureau of Professional and Occupational Affairs.
- Must demonstrate knowledge of aging process, possess the ability to assess the resident’s age-specific needs, and provide the care needed as set forth in Bradford Ecumenical Home policies and procedures.
- Must possess excellent computer skills. MDS and Wound Care experience preferred.
