Description

A certified community health worker contributes to improving health outcomes by serving the community in which they reside or share a similar ethnicity, language, socioeconomic status, or life experience. The term “community health worker,” includes but is not limited to other titles such as outreach worker, patient navigator, and promotores de salud.

Tasks

  • Maintain updated client records, including care plans, notes, and required documentation.
  • File appropriate forms and ensure client information is accurate and up to date.
  • Advise clients on ways to improve general health through diet, exercise, or routine care.
  • Educate community groups on health topics like nutrition, fitness, and wellness practices.
  • Identify individuals in high-risk or targeted groups needing support or outreach.
  • Contact members of minority, low-income, or pregnant populations for services or referrals.
