Description
Responsible to provide person-centered services to individuals with disabilities primarily focusing on social, safety and everyday living skills. Individuals within the program work daily with activities that encourage independence, self-esteem, self-advocating, physical and mental wellness and self-sufficiency.
Tasks
- Provide program support in absence of leadership; assist with new staff training.
- Deliver person-centered care per ISP in both site-based and community settings.
- Coordinate and facilitate community outings and integration opportunities.
- Ensure participant safety, personal care, and supervision as detailed in the ISP.
- Complete required documentation and incident reports per program regulations.
- Maintain a clean, safe program space and support a respectful team culture.
- Collaborate with staff to meet individual goals and fill open shifts as needed.
Qualifications / Education
- High school diploma or GED; preferred a minimum of five years’ experience working in a direct care position or with individuals with disabilities; or a combination of education and experience.
- Support program management by ensuring the provision of high quality supports and services. Ability to read and understand Individual Support Plans and schedules. Demonstrate excellent written, verbal, and organizational skills. Maintain a professional attitude and appearance. Strong and independent decision-making skills. Must be dependable and flexible. Ability to work in a team environment with all levels of the organization. Ability to apply basic first aid skills. Must have reliable transportation.
