Description

Responsible to provide person-centered services to individuals with disabilities primarily focusing on social, safety and everyday living skills. Individuals within the program work daily with activities that encourage independence, self-esteem, self-advocating, physical and mental wellness and self-sufficiency.

Tasks

  • Provide person-centered supports and supervision as outlined in each participant’s ISP.
  • Ensure participant safety, personal care, and assistance with daily living tasks.
  • Promote community integration and facilitate outings based on participant needs.
  • Accurately complete documentation and report incidents per program regulations.
  • Support various staffing ratios (1:1, 1:3, 1:6) and fill in open shifts as needed.
  • Maintain clean, safe program areas and collaborate regularly with the department.
  • Uphold agency policies and model respectful, professional behavior at all times.

Qualifications / Education

  • High school diploma or GED; preferred a minimum of one years’ experience working in a direct care position or with individuals with disabilities; or a combination of education and experience.
  • Ability to read and understand Individual Support Plans and schedules.
  • Demonstrate written, verbal and organizational skills.
  • Maintain a professional attitude and appearance.
  • Strong decision-making skills.
  • Must be dependable and flexible.
  • Ability to work in a team environment with all levels of the organization.
  • Ability to apply basic first aid skills.
  • Must have reliable transportation.
