Direct Support Professional II - In-Home and Community

Description

Responsible to provide person-centered services to individuals with disabilities across the areas of programming to include, In-Home and Community, companion, and respite. Services to be provided either in the home of the individual or within the community.

Tasks

  • Deliver person-centered supports across all agency programs per scheduled assignments.
  • Follow ISPs to provide appropriate supervision, care, and individualized support.
  • Ensure participant safety, health, and assistance with activities of daily living.
  • Promote inclusion through community integration and employment opportunities.
  • Complete documentation accurately; report incidents per program guidelines.
  • Keep program areas clean, safe, and organized for participant use.
  • Maintain professionalism; uphold agency policies and foster respectful interactions.

Qualifications / Education

  • High school diploma or GED; preferred a minimum of one years’ experience working in a direct care position or with individuals with disabilities; or a combination of education and experience.
  • Ability to read and understand Individual Support Plans and schedules.
  • Demonstrate written, verbal and organizational skills.
  • Maintain a professional attitude and appearance.
  • Strong decision-making skills. Must be dependable and flexible.
  • Ability to work in a team environment with all levels of the organizations.
  • Ability to apply basic first aid skills.
  • Must have reliable transportation.
