Description
Responsible to provide person-centered services to individuals with disabilities in a wide variety of programming areas to include but not limited to In-Home and Community, companion, vocational, community, employment, and adult training facilities.
Tasks
- Provide person-centered support across Work Center, Blaisdell, and IHCS programs.
- Follow ISPs to ensure appropriate supervision, care, and service delivery.
- Support participant health, safety, and daily living needs in all settings.
- Accurately document services and report incidents per program regulations.
- Promote inclusion through community-based activities and employment support.
- Keep program areas clean, organized, and safe for all participants.
- Uphold agency policies and model respectful, professional behavior at all times.
Qualifications / Education
- High school diploma or GED; preferred a minimum of one years’ experience working in a direct care position or with individuals with disabilities; or a combination of education and experience.
- Ability to read and understand Individual Support Plans and schedules.
- Demonstrate written, verbal and organizational skills.
- Maintain a professional attitude and appearance.
- Strong decision-making skills.
- Must be dependable and flexible.
- Ability to work in a team environment with all levels of the organizations.
- Ability to apply basic first aid skills.
- Must have reliable transportation.
